The Challenge: Silos Still Exist

Despite years of planning and good intentions, systems remain siloed. San Diego's most vulnerable residents—students with disabilities, English learners, foster and homeless youth, low-income families—still navigate fragmented care systems.

What CIS Was Built To Do

 CIS exists specifically to build Systems of Care infrastructure AND No Wrong Door service delivery. We do both the complex, long-term work of aligning partners across sectors and create the actual operational services that families can access immediately. 

Why Both Are Essential

 Systems of Care without service delivery: Great plans and MOUs, but families still can't find help when they need it.

Service delivery without Systems of Care: Individual programs that can't coordinate, share data, or create seamless pathways.

California has never had both working together in one integrated model. Until now.

The CIS Integrated Approach

 We build both the Systems of Care infrastructure and the No Wrong Door service delivery simultaneously.

We continue doing the hard alignment work with system partners—the governance structures, the MOUs, the cross-sector coordination, the data integration, the workforce training.

And we work hand-in-hand with community members to build Bridges to Care in North County San Diego—providing immediate respite care, navigation, and wraparound supports that families desperately need.

Bridges to Care is where both come together—the Systems of Care infrastructure enabling coordinated delivery of actual No Wrong Door services that families experience. It's California's first complete, integrated model serving real families in a real community, right now.
