Conduct an internal review of current infrastructures, existing data, initiatives, services, and programs to identify issues and generate potential solutions that assist in determining the needs and priorities process.
Stakeholders will build a shared culture, infrastructure, and processes that will endure and make a lasting impact. A shared vision creates a sense of community and, along with culture, is a core enabling factor for success.
Stakeholders will prioritize issues identified during the assessment and analysis phase. Two sets of criteria will be implemented: one for identifying the "North Star" community and contributing outcomes, and the other for determining the most effective strategies and approaches.
All stakeholders will align support and services to family needs and community partnerships by co-designing and implementing a single system of service delivery grounded in prevention.
